Accident hotspot fears after woman killed in A4137 crash | Hereford …
The A4137 was closed for several hours on October 18 after the crash, which involved a car and a lorry, at the Thorn House Farm crossroads.
The air ambulance, two ambulances, and a paramedic officer rushed to the scene alongside police and firefighters from Hereford and Ross-on-Wye.
A West Midlands Ambulance Service spokesperson said at the time that crews had discovered the female driver of the car in critical condition.
“She was already receiving basic life support from bystanders before ambulance staff began administering advanced life support," the spokesperson said.
“However, despite everyone’s best efforts it sadly became apparent nothing could be done to save the woman and she was confirmed dead at the scene."
Two men from the lorry were assessed and discharged at the scene, they confirmed.

A recent meeting of Llangarron Parish Council heard that the chairman had received communication from Herefordshire Council's senior accident investigation engineer Simon Hobbs.
Meeting minutes revealed that concerns were raised that the accident appeared to be being treated in isolation "although it is known that the crossroads is an accident hotspot".
The minutes said it was agreed that signage is poor when approaching from Llangarron and the A40.
It was agreed that the chairman would write to Mr Hobbs expressing "deep concerns" about the safety of the junction and asking that it be addressed as a matter of urgency.
